Faculty forms and reports

Many of the forms below may be filled in electronically, and then printed. Whether you can also save an electronic copy for reference will depend, however, on the set-up and installed software on the computer you are using. We would naturally be grateful if you could type in your information before printing the form, as the gain in legibility of your form would make it more efficient for all concerned. Do not quit before printing what you have written unless you are confident that you can save the document.

Feedback and training application forms for students

  • The University's new on-line Graduate Supervision System (GSS) gives all students enroled on graduate programmes the opportunity to report on their learning experience each term. Using the system is easy and intuitive, and we hope you will find that stopping for a few minutes and taking stock of what you have achieved in a termly cycle helps you to stay focused. The Humanities Division has published a short on-line guide with suggestions how the system could be most fruitfully used.
  • Simple questionnaire which allows you to report on your experience with series of lectures, classes, or seminars, normally to be returned to the individual lecturer or class convenor.
  • Sometimes students may find it helpful to receive not just informal comments but written feedback from their peers. This short grid of class or seminar presentation criteria is designed to facilitate peer feedback to speakers. Students giving any kind of presentation are encouraged to take the initiative in printing these off, distributing them to the audience, and learning what they can from the result.
  • In their second year of research, students are expected to submit a short progress report to the Faculty, but students on both taught and research programmes may find this form useful if they wish to draw some aspect of their experience to the Faculty's attention.
  • Form for candidates commencing a graduate programme in History which allows students and supervisors to assess initial training requirements.
  • Form providing basis for a regular discussion of training needs between student and supervisor throughout a student's post-graduate career.
  • Pre-registration form for language training.
  • The Faculty is continuously reviewing its provision of master's programmes, and any feedback you can provide on them is most welcome, whether you prefer a free-flow comment, or would be happy to answer our questionnaire (available as a DOC file), both should be send to the History Graduate Office, for the attention of the Coordinator of Graduate Studies.
  • Circular and registration form for training and gaining of experience in teaching undergraduates (HGO.91), usually available for second-year research students.
  • Report on mentored teaching experience (HGO.92).

Research-related forms and information

  • Interviewee consent form: this form helps researchers whose project involves interviewing individuals, either as witnesses to or as participants in historical events, to explain their project and the purpose of their investigation to their interviewees, and to obtain their written consent for appropriate and responsible use of their information in the project; it also helps to ensure that information is handled in compliance with current data protection legislation.
  • The University Safety Office provides valuable advice if your research takes you out of Oxford, particular for travel abroad and prolonged fieldwork: this includes information about cover through the University's travel insurance, and helpful checklists for the preparation of fieldtrips.   • Application for admission as a Recognized Student, with notes of guidance.
  • Form requesting a letter of introduction to archivists
  • Form requesting an extension of a current student visa (please ensure you leave sufficient time for the Faculty to process your documentation  before your appointment with the UK Border Agency and /or the expiry date of your current visa).
  • Outline of the examination process for research degrees (M.Litt. and D.Phil.) and a short overview of the adminstrative processes involved.


Forms for staff: assessment of and feedback to students

  • Class leaders, particularly in the case of compulsory classes in master's programmes are asked to send an attendance report to the History Graduate Studies Office (for submission to the relevant board of examiners). The report form is laid out in such a way that the class leader can decide to submit a summary report on all class participants, or to submit individual reports on each student.
  • Feedback to students about class presentations in taught programmes. Also available is a spreadsheet for Class Convenors on which they should record student presentations throughout the term for the information of examiners.
  • Feedback to students about seminar presentations in the course of a student's research training programme, and report to the Faculty's Graduate Studies Committee.
  • Student speakers have been encouraged to distribute a grid of criteria to their fellow students which will hopefully generate helpful feedback from their peers.
  • Feedback to students and report to the Faculty's Graduate Studies Committee about a student's performance in the Transfer and Confirmation of Status assessment process (HGO.2&14).

 

Forms for staff: proposal for a new paper, supervision reports, special tuition claims, etc.

  • New paper proposal form (HGO.80) – if possible return first version to Graduate Office by week 0 of Hilary Term of year prior to that in which paper is to be taught.
  • Timetable and procedures for changes in Examination Regulations.
  • An updated Graduate Supervision System (GSS) has gone live University-wide at the end of Michaelmas Term 2008. Current supervisors will by now have received several invitations to use the system, and it is accessed through the Single Sign On credentials you use for instance for your WebMail and WebLearn accounts. This reporting tool is designed in such a way that it allows students to comment themselves on their progress, training, and future training needs in advance of the submission of your supervision report. We would be grateful if you would encourage your students to make use of this opportunity (the Humanities Division has published some guidance for students intended to assist them in making effective use of this opportunity).
  • Supervisor's standard fee payments are generated through GSS as soon as supervisors have submitted their reports. – Additional payments, to you or one of your colleagues, relating to Special Supervision/Tuition and class teaching for graduates should be claimed separately through the Faculty. We provide a simple claim form for submission to the Faculty's Graduate Office, so that with the authorization of the the Director of Graduate Studies a payment can be processed. For your report to the student's programme convenor or supervisor you may wish to use our assessment form which you can complete electronically and then print for the convenor and your records (you are encouraged to submit a copy to the Faculty's Graduate Office for inclusion into the student's file).
  • If you have asked a colleague to provide special tuition or class teaching for your student, please send them an e-mail with this link for their report and any justified claims for payment.
